Ultrasound changes what can be seen in the room.

Musculoskeletal ultrasound uses high-frequency sound waves to visualize many joints, tendons, ligaments, muscles, bursae, and peripheral nerves in real time. Unlike static imaging, ultrasound can also evaluate structures dynamically while the patient moves or reproduces symptoms.

For me, ultrasound is not simply a way to guide a needle. It is part of the diagnostic examination when it can answer a focused clinical question.

Where it helps

Common musculoskeletal ultrasound applications.

Shoulder

Rotator cuff & biceps

Tendon structure, dynamic impingement, bursae, biceps pathology, and selected joint findings.

Elbow

Common flexor/extensor tendons

Tendinopathy, partial tears, enthesopathy, ulnar nerve, and other superficial structures.

Knee

Effusion & superficial structures

Synovitis, tendon problems, cysts, collateral structures, and procedural targets.

Ankle / foot

Achilles & plantar fascia

Tendons, fascia, ligament structures, and selected nerve entrapments.

Nerve

Peripheral nerve imaging

Median, ulnar, radial, tibial, peroneal, and other nerves can often be followed along their course.

Procedures

Image-guided treatment

Visualizing the target and needle in real time helps match the procedure to the intended anatomy.

Ultrasound and MRI

They answer different questions.

Ultrasound does not replace MRI, X-ray, or CT. Each modality has strengths. MRI is often better for deep intra-articular structures, bone marrow, and global joint assessment. Ultrasound excels at superficial soft tissue, dynamic assessment, rapid side-to-side comparison, and real-time procedural guidance.

The useful question is not which imaging test is “best.” It is which test answers the clinical question in front of us.

Common questions

What patients usually want to know.

What does RMSK mean?

RMSK stands for Registered in Musculoskeletal Sonography, a credential focused on musculoskeletal ultrasound knowledge and competency.

Can ultrasound replace an MRI?

Sometimes ultrasound can answer the clinical question without MRI, but the two modalities are complementary. MRI remains important for many deep, intra-articular, and bone-marrow problems.

Why use ultrasound for an injection?

Ultrasound allows the physician to visualize the target, surrounding structures, and needle in real time rather than relying only on surface landmarks.

Where does Dr. Girmann perform musculoskeletal ultrasound?
