A different first step

Before choosing a procedure, define the problem.

Nonsurgical orthopedic care works best when the diagnosis is specific enough to guide the next decision. My clinical approach starts with the history, examination, relevant imaging, and - when useful - diagnostic musculoskeletal ultrasound.

The goal is not to keep every patient away from surgery. It is to determine whether surgery is actually necessary, whether a more targeted nonsurgical strategy makes sense first, and what treatment best matches the tissue and the patient’s goals.

Localize the pain generator.Separate the structure that hurts from the structure that is merely abnormal on imaging.
Define the tissue problem.Joint, tendon, ligament, muscle, bone, bursa, or peripheral nerve - each behaves differently.
Match treatment to diagnosis.Rehabilitation, activity modification, medications, image-guided procedures, PRP, BMAC, or surgical consultation are tools - not default answers.
When this approach is useful

Patients often come after the first answer was not enough.

01

Persistent joint pain

Knee, shoulder, hip, elbow, ankle, wrist, or other joint symptoms that continue despite initial care.

02

Chronic tendon pain

Tendinopathy, partial tears, enthesopathy, recurrent flares, or symptoms that have not responded to ordinary rehabilitation.

03

Unclear diagnosis

Symptoms that do not fit the MRI report, or multiple competing findings that need clinical correlation.

04

Considering surgery

A second look at nonsurgical options before committing to an operation when the situation is not urgent.

05

Peripheral nerve symptoms

Entrapment and irritation evaluated with anatomy, examination, and ultrasound when appropriate.

06

Active patients

Athletes and active adults who care about preserving capacity, training, mobility, and long-term function.

Common questions

What patients usually want to know.

Do I need to have failed surgery or physical therapy before being evaluated?

No. Patients are seen at many points in the process - from a new injury to a chronic problem that has already been treated elsewhere.

Does nonsurgical orthopedics mean surgery is never recommended?

No. A good nonsurgical evaluation should also recognize when surgery is the best option and help the patient understand why.

Can ultrasound be performed during the visit?

Musculoskeletal ultrasound is frequently incorporated when it can answer a useful diagnostic question or improve procedural planning.
