Platelet-Rich Plasma

PRP in Cincinnati.
More than an injection.

PRP is most useful when the diagnosis, biologic preparation, dose, and procedural target are all treated as part of the same clinical decision.

PRP is not one thing

The syringe matters. The diagnosis matters more.

Platelet-rich plasma is an autologous blood product used in musculoskeletal medicine to deliver a concentrated platelet preparation to a targeted tissue. But “PRP” is not a single standardized product. Preparations can differ in platelet concentration, leukocyte content, red-cell contamination, volume, activation, and final dose.

That is why I do not think the useful question is simply, “Does PRP work?” The better questions are: for what diagnosis, in what tissue, using what preparation, at what dose, placed where, and compared with what alternative?

Common use cases

Where PRP is often considered in MSK practice.

Joint

Knee osteoarthritis

A common evidence-supported use case, with patient selection and disease severity influencing expectations.

Tendon

Chronic tendinopathy

Examples include common extensor, patellar, Achilles, gluteal, and selected rotator cuff tendon disorders.

Soft tissue

Selected injuries

Some partial tears and chronic enthesis-related problems may be considered after diagnosis and rehabilitation strategy are reviewed.

PRP should not substitute for diagnosis.

If the painful structure has not been identified, a biologic injection can be technically perfect and still target the wrong problem.

My approach

Four questions before a PRP procedure.

Is the diagnosis appropriate for PRP?Imaging abnormalities alone are not an indication.
What PRP phenotype is appropriate?Leukocyte-rich and leukocyte-poor preparations are not interchangeable in every tissue.
What is the biologic dose?Platelet concentration and volume together determine the number of platelets delivered.
Can the target be reached accurately?Ultrasound guidance helps place the product at the intended joint, tendon, enthesis, or other target.

What patients usually want to know.

Is PRP the same at every clinic?

No. PRP products can vary substantially in platelet concentration, leukocyte content, red blood cell contamination, total volume, and platelet dose.

What conditions does Dr. Girmann use PRP for?

PRP may be considered for selected joint and tendon disorders, including knee osteoarthritis and chronic tendinopathies, after the diagnosis and alternatives are reviewed.

Is PRP always performed with ultrasound?

For many musculoskeletal targets, ultrasound guidance is used to improve anatomic precision and confirm placement at the intended structure.
